Grant Guidelines Eligible Applicants: 501c3 nonprofit organizations located in Cuyahoga County, Ohio. Purpose and Activities: Deaconess Community Foundation will consider grant requests from 501c3 nonprofit organizations for:
Proposals that are of greatest interest to Deaconess Community Foundation are those have the strongest fit to our mission statement, which is “Deaconess Community Foundation provides resources that help organizations empower people to become self-sufficient. The Foundation’s actions are guided by the spiritual traditions of the United Church of Christ." Proposals must have some or all of the following characteristics:
In lieu of a financial grant, Deaconess Community Foundation has the internal capacity to provide accounting services to a limited number of qualified non-profit organizations for a specified time period. Grant requests for the following will not be considered:
A Proposal Request Summary form must accompany all requests for funding. The following information must either be included or attached: 1. Information on the institution or organization submitting the proposal, its' Mission Statement, and activities and/or research which have led to the proposal and the need for its implementation. 2. A brief overview of the proposal, including the amount and uses of money requested. 3. A complete project description, including objectives, steps and timetable for implementation (operational plan), qualifications of key personnel to be involved, and method of project evaluation. 4. A statement as to how the proposal request is a fit to the mission statement of Deaconess Community Foundation. 5. Internal Revenue Service determination letter(s) confirming the 501(c)(3) status classification of the applicant and a statement that your tax exemption has not been revoked or modified. 6. Most recent audited financial statements for the organization. Please ensure that you send an audit report a compilation or review is not acceptable. 7. Most current budget for the organization. 8. A current listing of your Board of Trustees, including company affiliation. Deaconess Community Foundation has four grant cycles per year. Dates that proposal requests must be received in the office are January 1 (first quarter grant cycle), April 1 (second quarter grant cycle), July 1 (third quarter grant cycle) and October 1 (fourth quarter grant cycle). If these dates fall on a holiday or weekend, proposal requests are due on the next business day. Decisions and disbursements of funds for proposal requests are made approximately three months from the due date. Proposals are screened and evaluated by staff. A site visit will be made by staff and Board Trustees for each proposal that is being considered. Written notification will be given for those proposals for which a site visit is not made because it is deemed to be outside of Deaconess Community Foundation's mission statement, guidelines and priorities. Site visit reports are prepared and reviewed by the Charitable Giving Committee at quarterly meetings. The Charitable Giving Committee makes recommendations for final approval by the Board of Trustees. Applicants are notified of Board decisions as soon as possible.
